New Patient Information
Our main goal will always be to treat our patients with respect while providing a warm and welcoming environment where they will receive the highest quality of dental care.
During your first visit to our office, you will be taken on a short tour of the facility and then you will meet with your doctor in a relaxed, private consultation room. Together, you and the doctor will discuss your needs and desires for your oral health. We generally recommend an examination and necessary x-rays to determine the condition of your teeth and any needed treatment. After your examination, the doctor will thoroughly explain what he’s found and what treatment is recommended. Questions about insurance and financial arrangements will also be addressed at this time by our treatment coordinator.
Please arrive 15 minutes ahead of your scheduled appointment time to fill out necessary paperwork. If you would like to complete your paperwork beforehand, you may print and fill out the appropriate forms found below.
We accept most dental insurance plans. Our staff will prepare all the necessary forms for your dental benefits. However, please remember that your specific policy is an agreement between you and your insurance company.
Please know that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated. Our treatment diagnosis is based on what is best for you. Your benefits are contracted between your employer and your insurance company.
Payment is expected at the time services are rendered, unless other arrangements have been made. This includes any co-payments or deductibles (amount not covered by your insurance). If you are concerned about cost, our doctors will design a customized treatment plan that can be scheduled according to your comfort level and convenience.
For your convenience, our office accepts cash, check, CareCredit, Visa, MasterCard, and Discover.
